
Affidavit of Domicile
If you were temporarily living with a friend or relative and need to enroll your child in school, you may be required to sign an affidavit of domicile
Signing the affidavit before Louisiana notary is similar to making the statement before a judge in that being untruthful could mean penalties.
The homeowner signs as the “Affiant” & the parent signs as the residing parent. Both must present IDs. The homeowner will likely need to show further proof of residence to the school board, but for the notary, you only need to prove your identity.
If the school board provides the document, be sure to wait to sign before the notary. If not, your notary will likely have a version for you to complete & sign in the office.
