Affidavit of Domicile
If you are temporarily living with a friend or relative and need to enroll your child in school, you may be required to sign an “Affidavit of Domicile”,
Signing the affidavit before a Louisiana notary carries a similar weight to making the statement before a judge. Lying on this document could mean fines or even jail time!
The homeowner signs as the “Affiant” and the parent signs as “Residing Parent”. Both must present IDs. The homeowner will likely need to show further proof of residence to the school board, but for the notary you only need to prove your identities.
If the school board provides the document be sure to wait to sign before the notary. If not, your notary will likely have a version for you to complete & sign in office.
